 Widely misattributed to Albert Einstein, this appears in Informal sociology (1963) by
William Bruce Cameron, an American sociologist
 
Winston Churchill didn’t say

Democracy is the worst form of government except all the others that have been tried.
What he said was, “It has been said that democracy is the worst form of government except all those other forms that have been tried from time to time” (my emphasis).
In other words, he was quoting someone else (in a speech in the House of Commons, 1947)
 

Insanity is repeating the same mistakes and expecting different results.
 ... comes from an anonymous pamphlet, Narcotics anonymous (1981), produced by the
Literature subcommittee of Narcotics anonymous.  Invariably misattributed to everyone from Confucius to Father Christmas.
