What exactly is a personal item and what makes it different than a carry-on?
Your personal item could be a purse, a briefcase, a camera bag (etc) and the general rule is that your personal item must fit under the seat in front of you. A carry-on will typically be a little larger bag that is stored in the overhead compartment.
Each airline treats personal items a little differently so it is worth checking with your airline to ensure your item will meet their guidelines. This is especially true now that some airlines are now offering lower cost flights but are charging for carry-on luggage.
