RECURRENT UTIâ€™s Part 3 ESTROGEN: what, exactly does estrogen have to do with this?
Estrogen increases vaginal epithelial glycogen by increasing the Lactobacilli that live in the vagina. That causes the production of lactic acid and hydrogen peroxide that is toxic to E. Coli. Post-menopausal women with a UTI can GREATLY benefit from topical estrogen to the vagina. Oral estrogens do not work as well for this purpose. In several well done studies, the incidence of UTI was decreased from 6 per year to 1 per year by the use of vaginal estrogen.
I recommend vagifem pellets two â€“three nights per week or Estring or topical Premarin or topical Estrace. Of course, this MUST be coordinated and approved by your gynecologist! Expect the topical therapy to take about 12 weeks to become effective. What about the long term use of antibiotics for prophylaxis?
While you are on the antibiotic, you will have fewer UTI. But, the moment you stop them, this protection goes away! In the sexually active women, a single post sex dose is as effective as taking it every day.50 mg of macrodantin or 100 mg of trimethoprim can be used.
IF YOU ARE ALREADY MY PATIENT, YOU WELL KNOW THAT I DO NOT LIKE TO GIVE PROPHYLACTIC ANTIBIOTICS. IT IS MY PREJUDICE THAT THE FEWER ANTIBIOTICS WE TAKE, THE BETTER. BUT, UNDER â€œDIREâ€ CIRCUMSTANCES, I WILL OCCASIONALLY APPROVE PROPHYLAXIS.
My clinical experience (no proven study) has shown that taking two cranberry pills around the time of sexual intercourse is as effective as prophylaxis with antibiotics.
A probiotic is live microorganisms that when given to patients confer health benefit. They restore normal flora (bacteria) A few, small studies show some benefit, but more work needs to be done, my clinical experience finds them VERY effective, especially in patients with any constipation or repetitive use of antibiotics. I like Culturelle every 12 hours , or VSL-3 once per day.
