KIDNEY STONE UPDATE 2016

DIETARY CHANGES ARE THE LEAST EXPENSIVE AND MOST HEALTHY WAY TO CONTROL YOUR STONE RISKS.
OSTEOPOROSIS AND HIP FRACTURES ARE MORE COMMON IN KIDNEY STONE FORMERS.
So, where does the calcium in the stones come from?
  1. Absorption from the bowels
  2. Re-absorption from bone
  3. Re-absorption from the kidney
  4. Disease states:sarcoidosis, Cushing’s syndrome, hyperparathyroidism, Paget’s disease
This is where the “brilliance of the body” comes in to play. If you limit calcium in your diet, you will absorb more oxalate and you will be MORE likely to form kidney stones. So, oddly enough, you need adequate calcium in food to prevent kidney stones that are made of calcium. (Most men need 1000 mg. of calcium in food per 24 hours and most women need 1200 mg in food per 24 hours)
One thing to always remember is that the body strives for “homeostasis”! Whatever you ingest, be it natural or pharmaceutical, the body will try to keep the parts that it needs and reject what it does not need. This is how we get unintentional consequences from things that we view as “healthy.”
So, the best way to prevent calcium stones is to have your urologist measure your blood and your 24 hours urine to determine YOUR risk factors and tailor your diet to your needs.
But, in summary, if you make calcium stones; be sure that you get your calcium in food. Food has stone blockers along with the calcium.(natural stone blockers are water, potassium, magnesium, phosphorus and citrate) If YOUR DOCTOR has prescribed calcium supplements and you are a calcium stone former, be sure that you take calcium citrate and that you take it after a high oxalate meal. (Eat a big salad for lunch and take your calcium supplement afterwards) This will bind the calcium that you do not need and expel it in your stool.
A few words about Vitamin D:
D2 is ergocalciferol. It is NOT produced by the body. It comes in supplement form.
D3 is cholecalciferol. It is made when the sun acts upon the skin and then the liver and kidneys change it into 1, 25 dihydroxyvitamin d3 (calcitriol), the active version of Vitamin D
The active version of Vitamin D (calcitriol) causes the bowels to absorb more calcium and the calcium to be absorbed from your bones, IF you are low in calcium.
Non stone forming patients can take normal doses of calcium and Vitamin D and have little risk for stone formation.
Kidney stone formers, however, need to have their Vitamin D levels measured and need to take their calcium as Calcium Citrate and take it with a big meal.  The best way for a kidney stone former to get vitamin D, is to drink a huge glass of lemon water and go in the sun with no sun screen for 20 minutes, three times per week.
